ICIS Changes!

In the coming weeks, the Unfractionated Heparin Dosing Protocol will be implemented in phases throughout the Hospital. When it is turned on at your unit, you will notice the following changes.

A new order set called the “Unfractionated Heparin Dosing Protocol” will be available when you search for heparin.

Unfractionated Heparin Dosing Protocol 

In the order set form, the routine Monitoring Parameters (such as labs and nursing instructions) are automatically checked-off for your convenience.

Monitoring Parameters 

Based on the appropriate therapy indication, select the Initial IV Bolus and Drip orders.

Heparin therapy indications 

Based on the patient’s height and adjusted weight, the Initial Bolus and Drip rate will automatically be calculated. If you need to make any adjustments to the rate, height or weight, you may open the corresponding Bolus or Drip order form.

If no adjustments are needed, click OK and Submit the orders.

When the orders are submitted, the Initial IV Bolus and Drip tasks are added to the nursing Worklist Manager. Nurses will be instructed to document and titrate the drip based on aPTT values.

You can view the patient’s progress in the Clinical Summary Tab, under the view Heparin Nomogram.
