Request Form for Clinical Decision Support

An alert is a message that will signal the presence of a potential or actual adverse clinical outcome in the context of a specific clinical situation. The alert generally interrupts clinical workflow, and is usually presented to the clinician during order entry or execution of another task, such as medication administration. Therefore, an alert might consist of an interruptive message that a particular patient is allergic to an ordered medication, and require an override, or might alert a nurse that the patient who is about to receive digoxin is hypokalemic.

Requests for implementation of a new clinical alert must be made by a hospital committee or department.
